This policy describes how this website handles information. It is not the same document as our HIPAA Notice of Privacy Practices, which describes how we handle protected health information as a pharmacy. If you are looking for your rights over your health records, that is the document you want.

What this site collects

This website does not set its own cookies, does not run analytics or advertising trackers, and does not build a profile of you. The only information we receive from this site is what you choose to type into the contact form.

Two third parties are involved in serving the pages you are reading, and each receives technical information such as your IP address as an ordinary part of delivering content:

• Our hosting provider, which serves this site and keeps standard server logs.
• Google Maps, which supplies the embedded map on our contact and about pages. That map is loaded from Google and is subject to Google’s own privacy terms.
• Twilio, which carries contact-form submissions to our team as a text message. It receives what you typed in order to deliver it, and is subject to its own privacy terms.

The contact form

If you use the contact form, the name, role, organization, email, phone number and message you enter are sent to our pharmacy team as a text message, through Twilio, our SMS provider. They are not stored in a database on this website.

Please do not enter health information, prescriptions or medical records into the contact form. Ordinary web form mail is not a secure channel. Send clinical information by fax or tell us on the phone instead — the numbers are on our contact page.

How we use what you send

We use it to answer you. We do not sell it, rent it, or share it with anyone outside Prime Infusions for marketing purposes.

Links to other sites

This site links to primeinfusions.com and to other organizations. We are not responsible for the privacy practices of sites we link to.

Children

This website is not directed at children and we do not knowingly collect information from children through it.

Changes

If we change what this site collects, we will change this page. The review date at the top tells you when it was last looked at.
